MDW - No Data Available

I've setup MDW for the first time, with data collectors on the same server as the MDW database. Disk usage, query statistics and server activity data collectors are running. SQL agent jobs are running as expected. There are no errors logged in data collectors log, SQL agent log, SQL log or Windows logs. Useful information is captured for the Disk Usage and Query Statistics reports. Server Activity displays info for Memory Usage and SQL Server Waits, but % CPU, Disk I/O Usage, Network Usage and SQL Server Activity indicate No Data Available. I've been through Troubleshooting the Data Collector (https://technet.microsoft.com/en-us/library/bb677178(v=sql.105).aspx) and various Google searches with nothing recognizable as a resolution. What are the next steps for troubleshooting?

EDIT:  There were indeed errors logged in Windows application log. For each instance of the perfmon counters collection, the following errors are logged.
